Very disturbed, the lead surgeon requests an independent investigation. The hospital turns to Dr. Taguchi (Yoko Takeuchi) who is a specialist in psychosomatic disorders who is shocked to at the assignment because she knows nothing in particular about surgery. Assured, however, that this very lack of insider specialized knowledge is in fact the "objectivity" required, she diligently begins to investigate in her own polite, but persistent manner.

Unlike the others in the series this film has a completely original storyline in which Kenshiro, near death after his battle with Shin and having used his remaining energy to kill a pack of wolves, was captured by slavers. Due to his strength, Kenshiro could escape whenever he wanted, but chose to stay and protect the other slaves.

This OVA features a somewhat original storyline because it presents the story from Yuria's perspective, spanning from her childhood, including the day she first met Kenshiro, to the conclusion of the previous film, as well as certain scenes from the manga and anime in which Yuria was not present. This story is the most expansive of the series, even covering never before seen events from Yuria's childhood, revealing both her mother and father, as well as her brother Ryuuga during his childhood. It also adds some details that were not in the manga, such as the time and place when Yuria developed her illness, her meeting with Rei (making his only appearance in the series), and her transition to the role of the Nanto General. One of the more notable additions is Yuria's dog Tobi, who serves an important role in the relationship between her and Ken. However, the OVA ends at the death of Souther and does not follow Yuria to the time of her death.

Tachibana Keiichiro (Shiina Kippei) is the owner; Ono Yusuke (Fujiki Naohito) is the superb baker that has a mysterious and mesmerizing aura; Kobayakawa Chikage (Abe Hiroshi) is the waiter and close bodyguard of Tachibana; and last but not least is Kanda Eiji (Takizawa Hideaki), the ex-boxer who loves cake so much that he becomes a waiter at 'Antique'. This is a refreshing, yet heartfelt comedy about the 4 very unique personalities that work until the wee hours of the night at the small confectionary. The series follows the romance, family life, and most importantly friendship that these 4 men from different generations experience. This is a confectionary with a "flavor" not like most. Located in the middle of an ordinary residential area, "Antique" stays open until 2 a.m. Every single thing in the shop is antique, in fact even the sherry glasses that water comes in look like they would cost 50,000 yen. On top of this, all the sweets that are sold are unbelievably delicious. The thing that is most peculiar about "Antique" though is that all 4 of the men that work here are very handsome, and each has an interesting past. Edogawa Rampo is a writer whose latest work is censored by the government, deemed too disturbing and injurious to the public to be allowed to be published. However, after burning his drafts, his publisher shows him a newspaper with an account of events just like his forbidden story. As the film progresses, fantasy and reality intermingle in a tale that draws heavily on influences from Poe and Stoker's Dracula. The film's strongly Expressionistic direction skillfully combines a variety of media (animation, computer-generated imagery, grainy black-and-white fast film stock, color negatives) for artistic effect.
