'Okkadu Migiladu' is the story of displaced Sri Lankan Tamilians who find themselves stuck with the tag of 'refugees' all their lives. The mistreatment they face both in India and Sri Lanka, with either country refusing to accept them as their own, forms the crux of the story.
Shourya (Manoj) completes his studies in the renowned Harvard University but comes to India to lead a life of a teacher. He helps the needy people. One chance meeting with Netra (Regina), a MP's daughter, turns into friendship and love blossoms. One day, they head to Kotilingala to do the jagaranam (staying awake at the night) in the temple. Late in the night, Netra's throat is slit by some unknown persons and immediately they take her to the hospital. Naturally Shourya becomes the suspect. What really happened? Who did this? Or has Shourya slit her throat as the police believe? Shourya (Manchu Manoj) is a highly educated individual who is in deep love with Nethra (Regina Cassendra). Due to unknown reasons, the couple decide to elope but an unfortunate series of events lead to Nethra being murdered. Twist in the tale arises when Shourya gets framed for the murder.
Guru Raj, a powerful business man of Charminar Group of Companies who runs his own gang as a mafia don, gets brutally assassinated. This incident upsets Guru Raj’s elder son Kaali and he decides to take revenge on his father’s killers. But twist in the tale arises when he too gets killed like his father. Shiva Rama Raju (Jagapati Babu), a noted good Samaritan in Parvatipuram who has a bet with Veerraju (Supreet) in the same village that "if any of his 3 daughters fall in love he will either kill them or cut down his ears" and he takes a vow that he will marry off his daughters to the grooms of his choice, neither love marriage nor elopement. Kavita (Rakul Preet Singh) is his third daughter. Raju (Manchu Manoj) is a wayward youth who is in deeply in love with an English teacher Sunny (Sunny Leone) in a junior college. Kavita is one of Sunny's students. Raju uses Kavita as courier to love Sunny; as the things proceed, Raju and Kavita fall in love. The rest of the story is all about what happens when they want to get married.
When Naidu confronts his son's rival group to end their squabbles, he encounters their mother Satya, his long lost love, separated by the twist of fate. On a trip to Bangalore, Kiran is kidnapped by a gang headed by Shajahan Bismil (Murali Sharma) and they demand a ransom of 2 Crores. In a curious twist of fate, the paths of both Nokia and Anu cross. They team up to save themselves and Kiran. How they are betrayed by the people they trust forms the rest of the story.
