A police officer is recruited to a special agency and given a license to kill. He is given a dozen or so young policemen and a bar as a front. Meanwhile, girls are disappearing and the friend of one girl is searching for her. It turns out the girls are being kidnapped and sold into bizarre kinky sex games to rich industrialists by a megalomaniacal corporation. One of the young deputies becomes involved and finds a friend of his (the boyfriend of the searching woman) is involved.

The local yakuza gangs dominating the port of Yokohama do not take well the invasion of their territory by a daring biker gang, led by a girl in black leather outfit and cowboy hat - reminiscent of the earlier war victors. She tracks down one of her classmates fathers, who runs an auto repair shop that the local Yakuza are trying to force out of business and take over, and starts working for him. At the same time a recently released from prison, and now ill Yakuza is trying to make a new life for himself and his girl, a friend of Reiko's, who also just graduated from reform school. A fateful car crash brings the two on a collision course with each other and the brutal Yakuza clan, which can only end bloody vengeance.

This film focuses on Okatsu; the adopted daughter of a master swordsman. She is a master with a sword herself and her talents far overshadow that of her brother, and real child of the man who adopted her. Her brother unfortunately has a gambling habit, and it plunges the family into trouble when he loses a lot of money in a crooked dice game. After releasing he is unable to pay the debt he owes; the blame is shouldered by the father, who is killed, leading Okatsu on a path of revenge.
An Osaka gangster Shimamura just got married. His new bride, Mineko is also involved in drug trafficking. When she goes to China to make a deal, things get botched pretty badly. Shimamura must travel to save her and recoup his employers’ losses.
Okatsu the Fugitive is the third film in the "Ohyaku/Okatsu" series. Okatsu; a "tomboy" who is good with a sword. Her father has found her a man to marry and actually she doesn't seem too upset about the situation. At the same time, a bunch of farmers are being slaughtered by a corrupt group running a tobacco smuggling ring. They're being investigated by a man who is documenting their crimes and when they find out about it, they torture him for information but get none. It's not long before he and Okatsu's mother wind up dead and she sets out for revenge.
